The form has a border made of four levels, from the outside: a border of white diamond shapes along a red band; enclosing a plain red line border; enclosing a border of fanciful foliage, geometric shapes and curls, which is enclosing another plain red line. Within this border set, in each of the upper corners is one quarter of a radiant sun. In the center of the form is a large tent with eight columns showing in the front and three men in long robes. Above and below the tent is printed the information of the form, which is filled out in black and blue ink. On the lower left corner is an impressed seal made of a triangle with the Chapter name and number reading up along the left leg, the words Royal Arch Masons reading down along the right leg, and illegible dates along the bottom leg. The symbols in the center of the triangle are illegible. |
